      Postsurgical fluid collections (PSFCs) can be a marked source of morbidity for patients, but advances in therapeutic endoscopy have allowed for endoscopic modalities of drainage to become more accessible.1 Prior studies have established the efficacy and safety of lumen-apposing metal stents (LAMSs) in PSFCs.1-4 In the video accompanying this case report (Video 1, available online at www.giejournal.org ), we demonstrate the transjejunal placement of a 6-mm LAMS for drainage of an infected postsurgical fluid collection.

      Bochdalek hernias are congenital disorders that result from the failure of the posterolateral diaphragmatic foramina to fuse properly.1 Most cases are diagnosed in children who present with symptoms of pulmonary insufficiency; however, Bochdalek hernias have also been described in up to 0.17% of asymptomatic adults.1,2 Rarely, adults may present with epigastric pain or obstructive symptoms from the herniation of intra-abdominal organs, such as the stomach, small bowel, colon, or omentum.3 We present a case in which EUS-guided gastroenterostomy (EUS-GE) was successfully used to relieve obstructive symptoms in a patient with a large right-sided Bochdalek hernia.

      A 64-year-old man with a history of diabetes mellitus, hypertension, chronic kidney disease, and primary mycotic infrarenal aortitis complicated with pseudoaneurysm formation after repair was seen at our hospital with a 3-day history of weakness and melena. His hemoglobin level on presentation was 6.1 g/dL from a baseline of 12.5 to 13 g/dL. International normalized ratio and platelet count were normal. Upper EGD was performed and revealed a large submucosal duodenal mass arising from the distal bulb to the second portion of the duodenum without involving the ampulla.
