      During the COVID-19 pandemic, direct contact between medical staff and patients in clinical practice (eg, endoscopy procedures) bears potential risk of infection. Telemedicine, or noncontact medicine, in this circumstance offers an ideal solution.1,2 Here, we report the first clinical experience of remote controlling capsule endoscopy for gastric examination.

      This case report video (Video 1, available online at www.VideoGIE.org ) aims to show and investigate the digestion process endoscopically. The subject of this experiment was the presenter, a healthy man in his 50s without any medication taken. The study was approved by the Ethics Committee of Nippon Medical School Chiba Hokusoh Hospital.       Magnetically controlled capsule endoscopy (MCE), a painless noninvasive screening tool for gastric diseases, has been an emerging diagnostic modality in clinical practice.1-7 Because the magnetic capsule in the stomach can be accurately controlled through multidimensional rotation and adaptive matching of an external C-arm robot, the diagnostic accuracy of MCE for detecting gastric focal lesions is comparable with that of conventional EGD.2,3

      A novel reusable endoscope (Olympus Corp, Tokyo, Japan) with an integrated motor (Fig. 1A) was developed for rotating a disposable short spiral overtube mounted on the insertion tube portion. The drive motor located in the endoscope handle is activated by foot pedals and controls the direction and speed of rotation of a coupler located in the middle of the endoscope’s insertion tube (Figs. 1B and C). The rotation coupler is the only portion of the endoscope that rotates; the distal and proximal portions of the insertion tube do not rotate.
